Supporting you in being you!
Simply Good Form’s Gender Affirming Name Change 2025 Program offers support for individuals seeking to change their name and gender marker.
Upcoming clinics will provide resources, including legal assistance for form completion, guidance for finger-printing and coverage of processing fees*. The program aims to provide a smooth pathway for updating their identity documentation for two-spirit, trans, and non-binary individuals. This is a volunteer-based event.
*Note: No money will be distributed at the event.
Once all materials are ready to be submitted, a cheque will be included with all documentation, by Simply Good Form Inc., for the fees and our team will process the final submission of materials.
Join us at an upcoming clinic. You can also schedule an appointment to begin the important journey of changing your name and/or gender marker. We warmly welcome two-spirit, trans, and non-binary individuals to receive the support they deserve during this significant process.
Our clinic is here to support you through the process of legal name and gender marker changes. We’re proud to have caring volunteers on site. They can compassionately witness the signing of your forms.
Location: The Pink Piano Café and Lounge
This is a queer-friendly space that is accessible with everyone washrooms.
SGF Inclusion Name and Gender Marker Program
At the Clinics, we have a volunteer legal team who will review your completed forms and sign them for you. Be sure to check the pathway for the forms and ID to bring for review. Forms will be onsite and we will have a team of volunteers to help you fill them in correctly.
Messages from You
And then there were the messages. Dozens of handwritten notes shared by participants—offering advice, encouragement, and solidarity to others still on the journey.
“It feels hard and overwhelming but it’s so worth it. Coming to a place like this makes us feel seen and heard.”
“This clinic was great and well appreciated! I have been trying to change my ID for years but the barriers of navigating paperwork alone and having to then have it notarized made the process nearly impossible to accomplish. Having everything in one place, facilitated by trans/queer folks made me feel held through the process and supported by community. Thank you for holding such an important space!” Kris R
“The name change process has always seemed like a big and daunting task, so it had been something I had been putting off for a little while despite the discomfort and frustration that comes with having to constantly correct people on my name. I could not even begin to figure out how to navigate it all between the fees, the confusing paperwork with easy-to-miss details, the fingerprinting, etc. I am thankful to everyone who made this clinic happen. I am especially thankful to Cyndi for coming with me to my fingerprinting appointment at the police station where she advocated for me and helped make things go a lot smoother than they would have if I went alone. I hope to see these clinics continue and help more people, including my friends whom I recommended should go to the upcoming one.” Clover
That’s what this clinic was about. Visibility. Care. Connection.
